An Excel spreadsheet would be really easy to do this in so that people could add to it and modify it.

Someone could get a good template going and go from there. The only problem would be file size. It would grow quite large. You would have to transfer the .xls via FTP or something. But you could .pdf it once it was complete for a simple solution.
