I've done this several times. Easy, just follow the instructions and fill out the DMV form from link above. You'll need to bring the following things to the DMV office.
New plates
YOM year sticker (in your case a 1969 sticker)
Old plates
Form filled out
current registration
checkbook

DMV will keep the form, your current plates, registration, and keep a photo copy of the new plates and 1969 sticker, and you check. They will give you a temp registration sticker and send you packing.

DMV will take about 4-5 months to send out new registration and stickers.
