First make sure all adjustments are correct.

There's a few possibilities. One is a bind in the input shaft housing where the throwout bearing slides back and forth. It could be bone dry and binding, have worn spots that keep it from sliding smoothly or the throwout bearing fork is not in the grooves correctly.

Other possibility is a defective pressure plate.
