Just went through this a few weeks ago with the same master cylinder/booster combo. I purchased a replacement line kit from Inline Tube and "conversion" lines that go from the master to proportioning valve. I then had to re-bend and flare the output lines from the proportioning valve. Here is a picture of it back together.
