GM trucks used a 34in frame width from who knows when right up to 1987 when they switched to the 37in everyone else was using. If I were to guess, that hitch was modified in some way to fit something. It looks like a regular class 4. I have a 72 with a 69 frame I put under it years ago. My "new" frame had 6in extensions on it, probably for a flat bed. I thought I bought a LWB frame but I had been told by a few that where it is a one ton, it was still a 127in WB. My point is that mods like that have to be dealt with before putting a class 4 hitch on.
When I cut the extensions off, I only had to drill out the front holes as I expected and shim the whole thing 1/4in. Works great now.
