I used the CPP setup. Fit and finish perfect, all the hardware you could need and more, comes with adjustable pushrod and prop valve too. Only had to drill two holes.
Cost about $239 plus freight. I have been buying CPP stuff from FleaBay instead of direct from CPP because it ships faster and it seems like customer service is better when PayPal and Ebay are breathing down their necks.

Truck Shop has a pre-bent brake line kit for this conversion for less than sixty bucks that fits GREAT. I had to get two reducers from the local hardware store to adapt the lines to the stock tees, but other than that it was flawless.
