49 chevy3100 disc brake conversion

Am looking at converting a 49 chevy 3100 to disc brakes. Need to change rear end because right now it has a 3/4 ton rear end in it and probably crazy high gears since it was used for a grain truck. I'm looking at getting a rear end from a 67-69 chevy c10 1/2 ton, so I would need a disc brake set up for that. Would also like to get disc brakes on the front. Planning on not changing the front end. I'm wondering if anyone could give me pointers if this seems correct to do or if there is a better set up out there? Also where should I buy a kit for the disc brakes? And since I'm looking to go through all that work to get disc brakes is there anything else I could replace to make everything work a lot smoother? Hope this makes sense...
