Looks like a Blazer with a cab back/roof added....
Edit: maybe not. That tailgate looks like a pickup gate (thin with no glass in it). K

I did that to a 78 blazer about 20 years ago , just cut the roof off and put a roof and the back part of the cab from a truck on. I also had a pickup tailgate on it and if my memory serves me correct I think the holes are there to bolt all the tailgate hardware on. I don't remember it being a hassle anyways.

No body break between the cab and bed is the easiest way to tell.
The unibody Ford trucks of the early 60's were like that, had to be careful crossing a good size bump or ditch, it was notorious for popping the back glass out. |

A buddy of mine has a really nice one that he built like that from a Blazer and the back part of a pickup cab. It is his daily driver, and looks really sharp.
