Fender mounted brake booster

Saw this at a local car show, I'm not sure how it works but the owner said it worked great,

Look similar to a midland style booster, normally found on C50 and up series trucks and are usually frame mounted.
I can understand why they used it, since the truck has combined brake/clutch master cylinder.
