Help... Sounds like a miss.

Okay, about 4-5 months ago my truck was running tip-top. Then reverse went out on my TH400. Then things seemed to snowball. Somehow a spark plug holder broke while i was driving one night, 7 miles from my house, and burnt three plug wires in half. I had no choice but to limp home, considering it was 3 am in a bad area to leave the 18k$ truck. So I limp her home. The next day I replace the spark plug wires. I take one off, and replace it exactly as it should be before continuing to the next. That would be the last time it really ran like it should. From that point on, when accelerating at low rpms, it sounds like it's missing a little. It sounds like its struggling so hard to get going. Once I'm at higher rpms, I can't hear it anymore. It may be the fact that I'm turning so many rpms that its not noticeable, but I'm not sure. A few nights ago I was driving it and noticed an exhaust leak, again... so right now I have the exhaust unbolted so it would be pointless to make a video with the obnoxious straight pipes. Anyone have any ideas? I'm really hoping that its possibly just a little off timing or a wire is switched on accident. Does it sound like anything else?

Edit 1: After checking, all my wires are correct, like I thought. Is it possible that this could be caused by a foul plug or wrong gaps?

Edit 2: After checking 3 of the plugs on the left bank, most are looking fairly old and caked up. I'm going to buy a set of plugs tonight and put them in in a few days. If this doesn't work, on to the cap/rotor.
