possible reason for door gap problem?...

My driver door has a problem where the gap between door and jamb is way too big. i have the top on and even with the door hinges all the way back there is still 3/16-1/4" too much gap. The part i don't get is the gap is even all the way down. We managed to get the passenger side even 3/16" gap top to bottom. I have new mounts, floors, etc. but its like the entire pillar is too far forward. I tried to put on the fender but it wont fit...This gap was also there before all the rust repair was done...any ideas? Frame out of allignment or tub somehow? Any ideas?
