Correct me if I'm wrong

Cruising the depths of marketplace for a local small block for a rebuild and I saw this. Ad says it's a 350 from an 80s pickup and I wondered why there was only 1 accessory hole in the heads. Then I saw the backside and I'm guessing they swapped sides with the heads for whatever reason or is there a breed of cylinder heads that are supposed to be like this? It's been a while since I rebuilt an engine
