Just how do the new cab bushings go?

So here is my current quandary: just how do the new cab-to-frame bushings go? What came out FELL INTO DUST and I couldn't see exactly how it went. The set from Classic Heartbeat lacks instructional diagrams. Can anyone help me out here?

Pics of how I THINK it might go. Please help.

This is driver's side front and rear bushing. The rear one was pretty nice actually.



*Note: no frame members indicated now in how these are now assembled.

Is this how the radiator core support bushings go?



Is this how the rear bushings go?



Is this right for the front bushings?



Does a flat washer go on top of the upper bushing? *So the top of the upper bushing doesn't contact the truck frame itself?

I know the original front bolt head rode directly on the cab floor....is there supposed to be a washer in between the cab floor and the bolt head?



What do all these pieces do?
Why the two different sizes of sleeves?
