I used the CPP Grand Slam kit with 13" rotors and kept the 6 lugs. This comes with springs and shocks, etc and went in with not too much trouble. I also bought the front brake line kit to complete the swap. Finding wheels was not easy, so consider that.

Pretty well all the big aftermarket companies offer similar stuff. Just consider customer support and price before you jump in.
