The old solution was a ball welded to a plate of 1/4 - 3/8" steel, bolted to the bed and support beams.
The newer setups have about a 4" hole in the bed, with a piece of channel underneath that the ball plugs into and the safey chains hook to. Pull out the ball and you have nothing protruding into the bed. Some include a cover plate that fits in the hole to prevent small items from falling through.
