Here is a Write up I did on AZKickin (Formerly A-Tron) Disc brake conversion kit. I'm pretty happy with it. They are in the process of designing a bracket for the parking brakes that should work with the stock brake cables.

It's one stop shopping. THe main kit includes machined rotors to fit 4x4 axle, Mounting plates. That will let you use either stock GM 4x4 front calipers or you can use Caddie rear calipers with the parking brake. Then they have sub kits including a hose kit that lets you retain the slock hardlines on the axle by just routing them to the hose. They also will sell you EVERYTHING in one shot (except the parking brak stuff because they still haven't finnalized the design).
