Were armrests ever available as an option for these cabs?

Yes, however you'll need to pull the door panels off to find the mounting nut locations and mark the door panels to drill access holes to mount them.

There were 2 different styles. The basic 1 piece molded, then the 2 piece which has a plastic color coded bottom and the vinyl top. My 65 Suburban only has 1 of the 1 piece units, on the driver door, that was originally fawn. Replacements are only black and red.
Can someone correct me if I am wrong on the 2 piece units?

My 65 Suburban has just the one on the drivers door and its the deluxe one. I have a set of door panels that have the factory holes in them from a 66 I scrapped. I use them as a template to ad holes to other sets.

I believe the drivers was required and the passengers was an option. Like said before, there was a standard and a deluxe armrest available.

You will also likely need to install the 1/4"-20 cage nuts into the inner door to anchor the arm rest to. If I remember correctly there are 2 sets of square holes in the inner door as possible locations to mount the arm rest.
